29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a) 42 Elizabeth St Pawtucket RI:   On or about June 24, 2022 the employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE) including but not limited to the use of eye and face protection for employees who were operating gasoline powered backpack air blowers, chain saws and towable wood chippers.  Abatement Note:   Once the PPE Hazard Assessment is completed you shall:     1) Certify that the hazard assessment has been performed through a written certification in accordance with 1910.132(d)(2),     2) Provide training to affected employees in proper use of required PPE 1910.132 (f)(1). 29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service:  (a) 42 Elizabeth St Pawtucket RI:   On or about June 24, 2022 the employer did not  did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service in that the safety line used by the tree climber was cut and  heavily abraded in several areas.  Abatement documentation is required per 29 CFR 1903.19, in the form of written (i.e. polices, procedures, photos and/or invoices) or other evidence that this item has been corrected.
