F Golden, CO 80403: On or before 6/21/22 the employer did not provide or require the use of hardware that would have controlled the potential energy of the blade chopping equipment during tactile blade checks. This condition exposed employees to amputation, laceration, and crushing hazards. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
