29 CFR 1910.212(a)(5):Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: a) Loc Performance Products LLC, DBA Loc Performance on or about June 21, 2022, at the address of 1115 South Wayne Street, St Marys, OH: The employer did not ensure the overhead industrial fan located in the press room line K23 was adequately guarded. In this condition employees were exposed to amputation hazards from the fan's metal blades.
