29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  JOB SITE: 814-824 Forest Avenue, Saint Charles, MO  63301, on or about 6/29/2022, workers were observed on top of the residential roof, actively installing new roof shingles, operating pneumatic nail guns without any form of eye or face protection. The workers were exposed to a struck by hazard.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE:  814-824 Forest Avenue, Saint Charles, MO  63301, on or about 6/29/2022, workers were observed on top of an 4:12 pitch roof, actively installing new roofing material, approximately 17 feet 2 inches eave to ground level, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  JOB SITE:  814-824 Forest Avenue, Saint Charles, MO  63301, on or about 6/29/2022, the employer did not ensure that each employee using a Louisville & Werner Aluminum Extension Ladders were installed properly to prevent a fall hazard. The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The workers were exposed to a fall hazard.
