29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or about June 13, 2022, the employer did not implement safe work practices when employees were engaged in activities including, but not limited to opening process equipment or piping to prevent workers from being exposed to chlorine dioxide while flushing process piping at the Bleach Plant. a) At ClO2 Pump #1, employees were exposed to splashing from the release of ClO2 liquid from the piping and the vapors emitted in the air from the ClO2. b) Employees connecting an industrial water hose to the exterior drainpipe of the ClO2 Pump #1 were exposed to ClO2 liquid released from the piping and vapors.
