29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing, labeling, or certification: At a worksite located at 85 Beirut Circle, Kansas City, MO 64153: Employees were exposed to electrical shock hazards. This was demonstrated when a relocated power tap (Tripp-Lite) was plugged into another extension cord in series and not in accordance with the listing, labeling, and certification of the extension cords. This occurred on or about June 24, 2022, and at times prior to that. The relocatable power tap was being used by employees for equipment.

29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: At a worksite located at 85 Beirut Circle, Kansas City, MO 64153: Employees were exposed to electrical shock hazards. This was demonstrated on or about June 24, 2022, and at times prior to, when a temporary light was connected in series to a cord and cable (extension cord), and the plug end for the portable extension cord was missing a ground pin.

This was demonstrated on or about June 24, 2022, and at times prior to, when a temporary light was connected in series to a cord and cable (extension cord), and the extension cord had been affixed to the structure in lieu of permanent wiring.
