29 CFR  1910.28(b)(3)(i):  The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items (A) through (D) of this section:  a) In the plant, around the Hydro Tester, employees were not protected from falling into a 12-foot by 28-foot rectangular floor hole, approximately 18-foot deep, surrounding the machine.

29 CFR  1910.28(b)(3)(ii):  The employer did not ensure each employee was protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  a) In the plant, employees were exposed to tripping hazards while operating the Okuma CNC machine Serial Number 227776 which had an unguarded floor opening approximately 240-inches long and 10-inches deep.   b) In the plant, employees were exposed to tripping hazards while operating the Okuma CNC machine Serial Number 227776 which had four unguarded 10-inch by 10-inch, 10-inch deep floor openings.  c) In the plant, employees were exposed to tripping hazards while operating the Okuma CNC Line 2 machine Serial Number 227777 which had four unguarded 10-inch by 10-inch, 25-inch deep floor openings.  d) In the plant, employees were exposed to tripping hazards while operating the Straightening Machine which had an unguarded floor opening 41-inches deep.
