29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about June 28, 2022, employees were exposed to fall hazards while working without fall protection from a pitched roof. The front of the house had a ground to eave height of approximately 17 feet. The rear of the house had a ground to eave height of approximately 26 feet and approximately 16 feet from the eave to the deck.  No abatement certification or documentation required for this violation.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about June 28, 2016, employees were using portable ladders which did not extend 3 feet above the upper landing being accessed.     No abatement certification or documentation required for this violation.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  a)  On or about June 28, 2022, the employer did not maintain records or certification of fall protection training for employees exposed to fall hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
