29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet (0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide: a) At the jobsite, employees were exposed to a fall hazard of approximately 28-feet while working from the fourth buck of a 5' x 7' tubular welded frame scaffold that had not been secured to the building.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder: a) At the jobsite, employees were exposed to a fall hazard of approximately 28-feet when they used a ladder attached to the scaffold that was not erected at least three feet above the fourth work platform of a tubular welded frame scaffold.
