29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Approximate middle of work site, 208 North Main Street, Forked River NJ: An employee was working and walking at the bottom of an unprotected trench containing unprotected sidewalls. The trench was approximately 10 feet deep and 17 feet wide in the location where this employee was exposed to struck by and cave in hazards. Condition noted on or about 06/30/22. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
