29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Gel Recycling Port Orange, FL; On or about June 13, 2022, at 2656 Tomoka Farms Road, Port Orange, Florida: An employee suffered a work-related injury on June 13, 2022 and was hospitalized. The employer was aware of the in-patient hospitalization on June 17, 2022, and did not notify OSHA until June 27, 2022.
