29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On July 30, 2022, employees used an extension cord to energize a Makita skill saw that had the outer sheathing pulled away exposing the internal wiring. The cord and internal wiring were not protected from strain and cuts exposing the employees to electrical shock hazards.
