29 CFR 1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On June 30, 2022, employees engaged in cornice work used pneumatic nail guns without wearing eye protection and were exposed to struck-by hazards and eye injuries.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On June 30, 2022, employees using a Makita skill saw with the power cord outer sheathing pulled away exposing the internal wiring and without a strain relief device were exposed to electrical shock hazard.
