29 CFR  1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a) Kenyon College located at 101 East Brooklyn Street Gambier, OH 43022:  On and preceding June 30, 2022, the employer did not ensure that procedures clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance  when employees were engaged in maintenance activities such as belt changes, lubrication and troubleshooting on machinery including but not limited to HVAC air handlers.  The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a) Kenyon College located at 101 East Brooklyn Street Gambier, OH 43022:  On and preceding June 30, 2022, the employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedures and the requirements of this standard were being followed. Employees were engaged in maintenance activities such as belt changes, lubrication and troubleshooting on machinery including but not limited to HVAC air handlers which required the use of hazardous energy control.  An inspection of the hazardous energy control program/procedures had not been performed to ensure adequacy and proper application. The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  a) Kenyon College located at 101 East Brooklyn Street Gambier, OH 43022:  On and preceding June 30, 2022, the employer did not provide training to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were being properly communicated.  Employees were engaged in maintenance activities such as belt changes, lubrication and troubleshooting on machinery including but not limited to HVAC air handlers which required the use of hazardous energy control.  Employees did not receive training that addressed applicable hazardous energy sources and the means/methods that needed to be used to isolation and/or control the energy and the means necessary for verification of effective energy control. The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a) Kenyon College located at 101 East Brooklyn Street Gambier, OH 43022:  On and preceding June 30, 2022, employees were exposed to machine hazards associated with moving parts during servicing and maintenance activities such as belt changes, lubrication and troubleshooting on machinery which included but is not limited to HVAC air handlers. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(S)(i)J, and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. The following condition exposed employees to caught-in and struck-by hazards.

29 CFR  1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    a) Kenyon College located at 101 East Brooklyn Street Gambier, OH 43022:  On and preceding June 30, 2022, the employer did not ensure that pulleys coming off of supply and return air handler motors were adequately guarded which exposed employees to caught-in hazards. Several air handler motors located in a utility room had exposed nip points due to guards that were inadequate, partially removed and/or falling off in areas that maintenance employees perform belt changes, lubrication and troubleshooting of the equipment.

Several air handler motors located in the utility room had exposed nip points due to guards that were inadequate, partially removed and/or falling off in areas  that maintenance employees perform belt changes, lubrication and troubleshooting of the equipment.
