Such material shall be properly secured to prevent its accidental movement: a) On or about June 29, 2022, and at times prior thereto, coverings for floor holes big enough for a person to fall through after duct work removal were not secured exposing employees to fall hazards. b) On or about June 29, 2022, and at times prior thereto, floor holes big enough for a person to step into or fall through from water pipe removal were not covered exposing employees to fall hazards.
