29 CFR 1904.4(a) as required by 29 CFR 1960.8(b): The Agency did not comply with the Occupational Safety and Health Administration standards applicable to the agency. The employer did not record each work-related illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent for the calendar year 2022: Location: Supervisor of Shipbuilding Office, Aegis Building, 574 Washington Street, Bath, Maine On or about 8/8/2022, the employer did not record at least three instances of employee work related COVID-19 infection on the OSHA 300 log or its equivalent: Instance #1. Employee #1 was exposed to COVID at work on or about 7/27/2022, tested positive on 8/5/2022 and lost 5 workdays. Instance #2. Employee #2 was exposed to COVID at work on or about 8/4/2022 and later tested positive on 8/7/2022 and lost 5 workdays. Instance #3. Employee #3 was exposed to COVID at work on or about 8/4/2022 and later tested positive on 8/6/2022 and lost 8 workdays. Abatement Certification and Documentation is required.
