29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about June 29, 2022, at a new residential construction site located at 7314 27th Street South, Fargo in North Dakota, an employee working in close proximity to someone operating pneumatic nail guns at approximately 120 psi and was exposed to eye and face hazards from flying particles while working on a residential roof without wearing impact resistant eye or face protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  (a) On or about June 29, 2022, at a residential construction site located at 7314 27th Street South, Fargo in North Dakota, the employee was allowed to continually use a ladder that did not extend three or more feet above the landing surface while installing roofing shingles.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) On or about June 29, 2022, where an employee was engaged in roofing activities on a residential construction site located at 7314 27th Street South, Fargo in North Dakota, the employee was exposed to fall hazards greater than 6 feet without a fall protection system in place.   The Golden Roofing & Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1552610, citation number 1 - 1 and was affirmed as a final order on October 22, 2021.
