On or about July 1, 2022, at the Montecillo Unit 3 excavation jobsite, an excavator was operated adjacent to an excavation was required to approach the edge of the excavation, the operator did not have a clear view of the edge of the excavation, and the employer did not provide a warning system such as barricades, hand or mechanical signals, or stop logs.
