On or about June 29, 2022, and at times prior thereto, employees feeding material by hand into in-running rollers on the back side of the C1 - 72-inch calendar exposing employees to caught-in and amputation hazards.

29 CFR 1910.216(c)(1): A safety tripod, cable, or wire center cord was not provided across each pair of in-running rolls extending the length of the face of the rolls. The safety tripping devices was not located within reach of the operator and the bite. On or about June 29, 2022, and at times prior thereto, an emergency e-stop cable was not provided across the in-running rollers on the back side of the C1 - 72-inch calendar and the existing cable between the top and middle rollers was not within reach of operators.
