29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about June 29, 2022: Employees engaged in millwright activities at 317 Road R, Shickley, Nebraska, were exposed to fall hazards in that personal fall arrest equipment was not inspected prior to use. An employee who was working on a grain bin that was 32 feet above the ground used a rope for a fall arrest system that was worn, frayed, and damaged.
