Location - 361 Ayer Road, Ayer, MA. On or about July 6, 2022, two employees were not protected from falling while working/walking on a second floor deck. The second floor deck was greater than thirteen feet above lower level.

Location - 361 Ayer Road, Ayer, MA. On or about July 6, 2022, employer did not provide training for an employee who was operating an all terrain powered industrial truck (PIT). The operator raised a load and left the PIT with load suspended greater than thirteen feet above lower level.

On or about July 6, 2022, a powered industrial truck operator raised a load and then left the cab of the PIT greater than twenty five feet with load suspended. The suspended load was greater than thirteen feet feet above lower level.
