Location - 17 Knowlton Dr, Acton, MA. On or about July 6, 2022, Instance A) Two employees were not protected while working at and around electrical lines while removing shingles off a roof. Instance B) Two employees were not protected while working at and around electrical lines while ascending and descending an aluminum ladder that was set up within three feet of live electrical lines.

Location - 17 Knowlton Dr, Acton, MA. On or about July 6, 2022, two employees were exposed to a fall hazard of at least sixteen feet above lower level from a residential roof where fall protection was not used.
