At 8771 Baymeadows Road, Jacksonville, Florida 32256: On or about July 6, 2022, the employer exposed an employee to eye injury hazards caused by flying debris, in that eye protection was not utilized by an employee, while operating a Makita saw to cut sheathing and a Hitachi nailer to nail sheathing.

At 8771 Baymeadows Road, Jacksonville, Florida 32256: On or about July 6, 2022, the employer exposed an employee to fall hazards ranging up to 24-four foot, in that fall protection was not utilized by the employee, while performing sheathing operations while in an aerial lift.
