29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person. Location - 17 Knowlton Dr, Acton, MA. On or about July 6, 2022, Instance A) The employer did not conduct frequent or regular inspection of the job site that would of identified two employees exposed to a fall hazard while working/walking on a roof. The employees did not set up a fall protection system to prevent them from falling from a height greater than sixteen feet above lower level. Instance B) The employer did not conduct frequent or regular inspection of the job site that would of identified two employees exposed to electrical hazard while using an aluminum ladder that was set up within approach distance of a live electrical feed to the residential house. The employees were also exposed to the live electrical while stripping the roof with a metal shingle remover within inches of the live electrical feed.
