29 CFR  1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about 7/6/22 at a manufacturing facility located at 701 Second Street, Monroe City, MO., employees were exposed to a struck by hazard. Workers were observed using dip pots to transfer molten 319 aluminum in excessive of 1,600 degrees Fahrenheit without wearing protective sleeves or FR rated jacket, insulated heat reflective silver coat, welders gloves, or a tinted face shield.

29 CFR  1910.132(d)(1)(i):The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  On or about 7/6/22 at a manufacturing facility located at 701 Second Street, Monroe City, MO., employees were exposed to a struck by hazard. Workers were observed using dip pots to transfer molten 319 aluminum in excessive of 1,600 degrees Fahrenheit without wearing protective sleeves or FR rated jacket, insulated heat reflective silver coat, welders gloves, or a tinted face shield as noted in their written hazard assessment. Also the CSHO observed an exposed employee actively working on the production shop floor without proper eye protection or hearing protection. The company hazard assessment states while in production areas safety glasses with side shields and ear plugs / ear muffs must be worn.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about 7/6/22 at a manufacturing facility located at 701 Second Street, Monroe City, MO., employees were exposed to a struck by hazard.  An employee was observed cutting material with an unguarded vertical band saw which was marked 06BS002. Video 0072 shows the employee was 0 - 12 inches from the running saw blade during the operation.

29 CFR  1910.1200(f)(6):  Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged, or marked.  On or about 7/6/22 at a manufacturing facility located at 701 Second Street, Monroe City, MO., employees were exposed to a chemical hazard. The employer failed to properly mark boron nitride, which was being stored and used from a plastic Gatorade drinking bottle. A supply table located out in the open on the shop floor had four plastic Gatorade bottles  which were all being used to store boron nitride. The SDS sheet states that after incidental ingestion to not induce vomiting, but contact a medical professional or Poison Control Center immediately.
