On or about July 7, 2021, on the warehouse floor, passageways, aisleways and walking-working surfaces were not kept in a clean and orderly condition due to pallets, boxes of product, equipment, trash, and/or other debris items which were being stored/staged in working areas and on the floor exposing employees to slip, trip, fall hazards and fire hazards due to a lack a lack of access and egress in the event of an emergency.

29 CFR 1910.37(a)(3):Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a. On or about July 7, 2022, the employer did not ensure that exits and exit routes were free and unobstructed. Emergency exit door number 15 was blocked by conveyor systems, pallets and boxes with product. The following condition exposed employees to fire hazards. b. On or about July 7, 2022, the employer did not ensure that exits and exit routes were free and unobstructed. Emergency exit door number 14 was blocked by pallets. The following condition exposed employees to fire hazards.

29 CFR 1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. a. On or about July 7, 2022, the employer did not ensure that portable fire extinguishers were readily accessible to employees for use in event of a fire. A portable fire extinguisher next to emergency exit door number 15 was obstructed by conveyor systems, pallets and boxes of product. The following condition exposed employees to fire hazards. b. On or about July 7, 2022, the employer did not ensure that portable fire extinguishers were readily accessible to employees for use in event of a fire. A portable fire extinguisher located at pillar marked A was missing. c. On or about July 7, 2022, the employer did not ensure that portable fire extinguishers were readily accessible to employees for use in event of a fire. A portable fire extinguisher next to emergency exit door number 14 was obstructed by pallets. The following condition exposed employees to fire hazards. d. On or about July 7, 2022, the employer did not ensure that portable fire extinguishers were readily accessible to employees for use in event of a fire. A portable fire extinguisher between dock doors 20 and 21 was obstructed by pallets and boxes. The following condition exposed employees to fire hazards.

29 CFR 1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. On or about July 7,2022, and at times prior, throughout the Warehouse area, employees operated forklifts to move materials in aisles/passageways that were not kept clear without obstructions nor were aisles/passageways permanently marked. The following condition exposed employees to struck-by hazards. NFI Industries was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1277160, citation number 1, item number 1 and was affirmed as a final order on January 9, 2018, with respect to a workplace located at 25 Rix Mills Road, New Concord, OH 43762.
