29 CFR 1926.251(a)(2)(ii): Rigging equipment was loaded in excess of its recommended safe working load as prescribed by the manufacturer. On or about July 7, 2022, a tow strap and chain combination failed when it was used to pull a skid steer loader that was stuck at a lower elevation exposing employees to struck-by injuries
