29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  A) On or about July 6, 2022, at 13050 Addilyn Ct., Lot #3, Davie, Florida, employees were exposed to a  fall height of approximately 13 feet to ground level when applying plastering to the walls of a home under construction when using a scaffold without a safe means of access and egress; the employees used the crossbraces to access/egress the scaffold.  B) On or about July 7, 2022, at 13050 Addilyn Ct., Lot #8, Davie, Florida, employees were exposed to a  fall height of approximately 13 feet to ground level when applying plastering to the walls of a home under construction when using a scaffold without a safe means of access and egress; the employees used the crossbraces to access/egress the scaffold.  C) On or about July 8, 2022, at 13050 Addilyn Ct., Lot #2, Davie, Florida, employees were exposed to a  fall height of approximately 13 feet to ground level when applying plastering to the walls of a home under construction when using a scaffold without a safe means of access and egress; the employees used the crossbraces to access/egress the scaffold.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  A) On or about July 6, 2022, at 13050 Addilyn Ct, Lot #3, Davie, Florida, employees were exposed to a  fall height of approximately 13 feet to ground level when applying plastering to the walls of a home under construction while not utilizing guardrails and/or any other means of fall protection.  B) On or about July 7, 2022, at 13050 Addilyn Ct, Lot #8, Davie, Florida, employees were exposed to a  fall height of approximately 13 feet to ground level when applying plastering to the walls of a home under construction while not utilizing guardrails and/or any other means of fall protection. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  A) On or about July 6, 2022, at 13050 Addilyn Ct, Lot #3, Davie, FL 33325, employees were exposed to a  fall height of up to 13 feet to ground level when plastering the wall of a two-story residential home while not utilizing a personal fall arrest system nor were they provided with an alternative fall protection system.
