The operator did not recognize damage to the electrical cord of the operation pedestal of the 500-ton Bliss Press, damage to the lights and buttons of the operation pedestal and the yellow reset switch on the back of the 200-ton Minster 1 press, and damage to the yellow control button on the operation pedestal of the 300-ton Minster 2 press.   b) On or about July 11, 2022, the employer did not ensure that the general component of the inspection program was being followed in that inspections were not being done and necessary maintenance and repair were not being completed for the 500-ton Komatsu press.
