On or about May 26, 2022, the employer did not ensure procedures were utilized for the control of potentially hazardous electrical energy of the D-Line horizontal conveyor, while employees were engaged in service and maintenance activities, such as but not limited to, adjusting and/or repairing the D-Line horizontal conveyor belt.

29 CFR 1910.147(d): The established procedure for the application of energy control was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about May 26, 2022, the employer did not ensure employees performing service and maintenance activities on the D-Line horizontal conveyor applied energy controls by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the electrical energy source [(d)(3)], affixing lockout device(s) to the energy isolation device(s) [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to adjusting and/or repairing the conveyor's belt.
