29 CFR 1910.305(a)(2)(x):Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. Employees were exposed to electrical shock hazards. Employees engaged in kitchen and waiting activities were exposed to electrical hazards. Extension cord powering a mobile air conditioning unit was exposed to fraying by being ran through a doorway. As a result, the extension cord was damaged and the sheathing was worn.

At Hawaiian Bros Island Grill 1501 W. 23rd St. Lawrence, KS. 66046 on or about July 11, 2022 the employer failed to provide copies of the injury and illness records to an authorized representative.
