OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards from exploding plastic (Polyvinyl Chloride (PVC) pipe:  On or about July 12, 2022, in the maintenance shop area, employees were exposed to struck-by hazards from explosion and sharp shards flying in the air from the use of plastic polyvinyl chloride (PVC) piping used to transport above ground compressed air to the for the Ingersoll Rand (Serial CBV719015, Model: TS4N5) Air Compressor.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  On or about July 12, 2022, on the west side of the maintenance shop next to the refrigerator, oxygen cylinders in storage were stored less than 3 feet from acetylene cylinders, exposing employees to explosion hazards.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating.  On or about July 12, 2022, in the maintenance shop, employees used a damaged electrical outlet to provide power to equipment such as, but not limited to refrigerators for employee use, exposing employees to electrical shock and burns.

29 CFR  1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:   On or about July 12, 2022, in the maintenance shop, employees used damaged flexible cord sets to provide power to equipment including, but not limited to portable hand grinders.  The cords were repaired with electrical tape.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about July 12, 2022, in the maintenance shop, employees used uncapped beverage containers for storage of unknown chemical products.  The containers were not labeled.
