29 CFR 1926.1052(b)(1): Except during stairway construction, foot traffic was not prohibited on stairways with pan stairs where the treads and/or landings are to be filled in with concrete or other material at a later date, unless the stairs are temporarily fitted with wood or other solid material at least to the top edge of each pan: 33 Jackson Rd., Devens, MA: On or about July 12, Stairways with pan stairs where the treads were to be filled at a later date with concrete or other material were filled with rigid foam insulation panels, a non solid material and not filled with wood or other solid material to the top edge of each pan.

29 CFR 1926.1053(b)(5)(i):Non-self-supporting ladders were used at an angle such that the horizontal distance from the top support to the foot of the ladder was greater than approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): 33 Jackson Rd., Devens, MA: On or about July 12, 2022, an employee was observed using a fiberglass extension ladder to reach the roof that was set at a nearly 4 to 3 angle instead of the required 4 to 1 angle.
