29 CFR 1926.451(e)(4)(i): A stair rail consisting of a top rail and a mid rail was not provided on each side of each scaffold stairway. Location: a) Supported scaffold on the exterior of the building: On or about July 11, 2022, a stair rail was not provided for a scaffold stairway connecting the second-tier scaffold platform and the ground level. An employee fell 14 feet to the ground when he used the stairway.

29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold. Location: a) Supported scaffold on the exterior of the building: On or about July 11, 2022, an employee was going from the second-tier scaffold platform to the ground level using a scaffold stairway. The scaffold stairway did not have stairrail. The employee lost his balance when he was using the stairway and fell approximately 14 feet from the stairway to the ground level.
