Location - 80 Bromfield Street, Watertown, MA. On or about July 13, 2022, five employees were working/walking near or at the approach distance of the live electrical service to the house. The employees were re-roofing a residential home.

On or about July 13, 2022, an employee was working/walking on a residential roof at time greater than sixteen feet above lower level. The employee was wearing a body belt with lifeline attached to the front not at the center of the his back between the upper shoulder blades. The employee re-roofing the front of residential house.
