29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, falling or flying objects, or electrical shock and burns, were not protected by protective helmets. a.) On or about July 11, 2022, on the north side of the site, an employee was working from the ground under and adjacent to material without a protective helmet. This exposed the employee to a struck-by hazard. b.) On or about August 9, 2022, on the east side of the site, employees were not in use of protective helmets when working from the ground under and adjacent to employees working with material/tools above. This exposed the employees to a struck-by hazard.

On or about July 11, 2022, on the north side of the site, an employee was working from an aerial lift without fall protection. This exposed the employee to a fall hazard. b.) On or about August 9, 2022, on the north side of the site, an employee was working from an aerial lift with fall protection improperly donned. This exposed the employee to a fall hazard.
