The employees were exposed to potential ricochets' from the nail gun and did not utilize eye protection.

The employees did not use or properly use fall protection and were exposed to falls of up to 12 feet eave to ground The Two Brothers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1420043, citation number 1, item number 1a and was affirmed as a final order on September 15, 2019, with respect to a workplace located at 958 Coleman Ave, Fairmont, WV.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) 216 Fairmont Ave, Fairmont, WV: On July 12, 2022 and times prior, employees were exposed to a fall of 12 feet from the roof eave to the ground below while replacing shingles on a commercial roof with a slope of 6:12. The employer has not provided employees with training on recognizing fall hazards and the procedures required to be protected from the fall hazard. The Two Brothers, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1420043, citation number 1, item number 1b and was affirmed as a final order on September 15, 2022, with respect to a workplace located at 958 Coleman Ave, Fairmont, WV.
