On or about July 13, 2022 at jobsite 3090 Braeburn Way, Woodstock, Illinois 60098, employees engaged in roofing activities were exposed to falls of approximately 20 feet or more above ground level while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.
