29CFR1926.403(i)(2)(i): Live parts of electric equipment operating at 50-volts or more were not guarded against accidental contact. Location: 754 Grand Street, Brooklyn, NY 11211 On or about 7/13/2022, a) At the west wall of a hallway on the ground floor, 110-volt live conductors protruding from junction boxes were not guarded from accidental contact by employees. b) At the east wall of a hallway on the ground floor, 110-volt live empty lightbulb sockets of the temporary lighting system were not guarded from accidental contact by employees. c) A junction box by the superintendent's desk was not completely covered. Live conductors of 110-volt were not guarded from accidental contact by employees. d) A breaker panel in the basement was missing the dead front cover. Live electrical parts of 110-volt were not guarded from accidental contact by employees.

Location: 754 Grand St. Brooklyn, NY. On or about 7/27/2022 a) Employees used a stairway where the top 6 steps were not fitted with treads to cover the openings of the steps.

On or about 7/13/2022 a) A stairway system consisting of approximately 12 risers and rising approximately 8 feet was not equipped with a handrail on either of its enclosed sides. Employees used it to access the work area in the basement level.
