29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  On or about July 13, 2022, at a work site located at 5015 Meade Street, Appleton, WI, employees reroofing a residential home were either not wearing personal fall protection or had not attached the donned fall arrest harness to an anchored life line.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: (i) as part of a complete personal fall arrest system which maintains a safety factor of at least two; and (ii) under the supervision of a qualified person:  On or about July 13, 2022, at a work site located at 5015 Meade Street, Appleton, WI, the base legs of two installed roof anchors were not flush with the roof and the roof anchors were installed with fewer than the required number of fasteners to the roof.

29 CFR  1926.503(a)(2)(iii):The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: (iii) The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  On or about July 13, 2022, at a work site located at 5015 Meade Street, Appleton, WI, employees displayed deficiencies in their knowledge related to personal fall arrest systems including, but not limited to: when fall protection is required, proper installation of roof anchors, and adjusting the position of the rope grab on the line line.
