On or about July 14, 2022, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operators performance, thereby exposing employees to struck by and crushing hazards.

On or about July 14, 2022, at the above address, the employer failed to ensure the employees who operate power industrial trucks including Linde lift truck, Model H70D-02, Serial's E1X353N0041170, E1X353M0117679; Linde lift truck, Model H70D-03, Serial 268549; Taylor Machine Works, Model TE-360S, Serial S-W1-26357; TCM lift truck, Model FHD36Z2T, Serial A34N00118 were reevaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.

The power industrial vehicle did not have a functioning horn and was not removed from service. b. On or about July 14, 2022, at the above address, employees were exposed to struck-by hazards, when required to operate Linde lift truck, Model H70D-02, Serial E1X353M0117679. The power industrial vehicle did not have a functioning horn and was not removed from service. c. On or about July 14, 2022, at the above address, employees were exposed to struck-by hazards, when required to operate TCM lift truck, Model FHD36Z2T, Serial A34N00118. The power industrial vehicle did not have a functioning turn signals/brake lights and was not removed from service.
