29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall.   On or about July 14, 2022, on the southwest side of the site, employees performing roofing work carried roofing supplies and tools in their arms or on their shoulders while climbing the ladders to access the roof. This exposed the employee to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about July 14, 2022, employees performed roofing work without fall protection from a roof greater than six (6) feet to the ground below. This exposed the employees to a fall hazard in several locations:         a.) On the South side of the site on building #G of the complex.          b.) On the Southwest side of the site on building #I of the complex.  Premier South Roofing & Sheet Metal LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1466238, citation number 1, item number 1 and was affirmed as a final order on May 1, 2021, with respect to a workplace located at 19010 Ponchatoula Parks Drive, Ponchatoula, LA 70454.  Premier South Roofing & Sheet Metal LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1325670, citation number 1, item number 1 and was affirmed as a final order on November 6, 2018, with respect to a workplace located at 5002 Claycut Road, Baton Rouge LA 70806.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   On or about July 14, 2022, employees used extension ladders to gain access to the roof, and the side rails of the extension ladder did not extend at least three (3) feet above the landing. This exposed the employees to a fall hazard. a.) On the South side of the site on building #G of the complex.  b.) On the Southwest side of the site on building #I of the complex.   Premier South Roofing & Sheet Metal LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1466238, citation number 2, item number 1 and was affirmed as a final order on May 1, 2021, with respect to a workplace located at 19010 Ponchatoula Parks Drive, Ponchatoula, LA 70454.  Premier South Roofing & Sheet Metal LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1527694, citation number 1, item number 1 and was affirmed as a final order on June 1, 2021, with respect to a workplace located at 6031 Jones Creek, Baton Rouge, LA 70816.  Premier South Roofing & Sheet Metal LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1418442, citation number 2, item number 1 and was affirmed as a final order on December 27, 2019, with respect to a workplace located at 4740 Bluebonnet Blvd, Baton Rouge LA 70809.
