29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about July 14, 2022, three employees did not have a means of egress such as a stairway, ladder or a ramp while conducting work in an approximately 30 foot long by 15 foot foot wide and 9 foot deep trench/excavation.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about July 14, 2022, three employees were exposed to cave-in hazards, and were not protected by an adequate protective system designed in accordance with 29 CFR 1926.652 (b). The bench system used to protect employees had an angle of repose of approximately 56.6 degrees that was greater than the maximum of 45 degrees that is allowed in Type B Soil.
