29 CFR 1926.453(b)(2)(v):A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: At a commercial worksite located at 900 N Forest, Liberty MO -- employees were working from an aerial lift with no fall protection and lanyard. This exposed the employee to being catapulted and tossed out of the aerial lift with fall hazards greater than 30 feet to ground level. This was documented on or about 07/12/2022.
