29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) At Beasley Forest Products, Inc., located at 712 Uvalda Hwy Hazlehurst, GA 31539: On or about July 6, 2022 and times prior to, employees were exposed to caught-in and struck-by hazards when the employer did not ensure that a written control of hazardous energy procedure was documented and employees utilized the procedure to lock-out the equipment such as, but not limited to, the Green Stacker and De-Scrambler before entering into the access-restricted areas where unguarded and potentially moving bar chain conveyors and in-running nip points were located.

29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) At Beasley Forest Products, Inc., located at 712 Uvalda Hwy Hazlehurst, GA 31539: On or about July 6, 2022 and times prior to, employees were exposed to caught-in and struck-by hazards when the employer did not ensure that employees affixed a lockout device to each energy isolating device for equipment such as, but not limited to, the Green Stacker and De-Scrambler before entering into the access-restricted areas where unguarded and potentially moving bar chain conveyors and in-running nip points were located.

29 CFR 1910.265(c)(26)(viii): Lower landing area(s) of stacker(s) and unstacker(s) were not guarded by enclosure(s) that prevented entrance to the area(s) or pit(s) below the hoist platform(s), nor were entrances protected by electrically interlocked gates which, when opened, disconnect the power and set the hoist brakes. *If an interlock is not installed, other positive means of protecting the entrance shall be provided.    a) At Beasley Forest Products, Inc., located at 712 Uvalde Hwy Hazlehurst, GA 3153: On July 6th and times prior to, the employer exposed employees to struck-by and caught-in hazards in that the lower landing areas of the Green Stacker were not guarded by enclosures to prevent unauthorized entry nor electrically interlocked gates which disconnect the power when opened.
