C & M Properties and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1592999, citation number 1, item number 4, group letter a, and was affirmed as a final order on July 11, 2022, with respect to a site located at 1919 Whistling Straits Dr, Altoona, WI 54720.  C & M Properties and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1592631, citation number 1, item number 1, and was affirmed as a final order on May 06, 2022, with respect to a site located at Lot No. 5 Trilogy Road, Eau Claire, WI 54701.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Southeast side of structure; An employee was operating two Dewalt cordless circular saws (Model # DCS570 and DCS575) to cut sheathing without the use of appropriate eye protection.  (b) Southeast side of structure; An employee  was operating a Paslode CFN325XP cordless framing nailer to install sheathing on a residential structure without the use of appropriate eye protection.  C & M Properties and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1592999, citation number 1, item number 2, and was affirmed as a final order on July 11, 2022, with respect to a site located at 1919 Whistling Straits Dr, Altoona, WI 54720.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  (a) Southeast side of structure; A Werner MT-22 twenty-two foot aluminum multi-position ladder placed at ground level and used as an extension ladder to access the roof, was not of sufficient height to allow for the side rails to extend at least 3 feet above the landing surface, was not equipped with a grasping device(s), and was not effectively secured to prevent displacement.
