On or about July 15, 2022 and at times prior thereto, the employer allowed employees to perform shingle installation without first ensuring employees were trained on the hazards associated with falls to include but not limited to setting up and inspecting fall protection exposing employees to a fall hazard of up to9 feet 4 inches to the ground below.
