On or about July 11, 2022, while at the facility, employees were performing maintenance on a tray making machine, during which time the employer failed to ensure that the employees met the authorized employee training requirements of this part, such as the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.
