29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about June 18, 2022 the employer did not ensure that procedures for the control of hazardous energies were utilized during service an maintenance of the Muller Martini Binder conveyor system. An employee removed a barrier guard in place covering a belt and pulley drive system for the conveyor without utilizing the LO/TO procedures of the equipment. Hazardous energies present include mechanical and electrical. b) On or about June 24, 2022 the employer did not ensure that procedures for the control of hazardous energies were utilized during service an maintenance of the #530 Sheridan Stitcher . Employees were performing a size change on the equipment without having utilized the procedures for the control of hazardous energies. Hazardous energies present on the equipment include mechanical, pneumatic, and electrical.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation; On or about June 24, 2022 the employer did not ensure that employees tasked with service and maintenance of equipment within the facility had received training as authorized employees for the control of hazardous energies. Employees are tasked with performing setup and changes to equipment for production without having received the required training.
