(a) Pier-H, USNS 1st Lt. Jack Lummus at the upper Exhaust Stack: On or about July 17, 2022 a "blank" constructed of plywood and fire resistant material was placed inside the exhaust stack to prevent slag and sparks from going  down the stack during welding operations. There were no markings or signs warning workers not to enter or stand on the blank which could be mistaken as a work solid platform.
